Minster Local Schools

The Auglaize County Health Department will be offering an on-site Immunization Clinic at Minster Schools on Friday, March 10.  A letter and consent form is being sent to your home via USPS.  Only students who return the completed forms will receive the vaccine.  These forms must be returned by March 2.  If you misplace your form or you don’t receive it, the forms have also been provided at this link for your convenience:   https://5il.co/1ou5r
